For your convenience, we also accept payments at the CADTP Counselors’ Payment Portal where you can pay fees and upload signed documents.
Instructions for Completing an Application Online
The Online Application Process
- Select the correct application process. Scroll to the correct category and select the appropriate tab.
- Click the link to the application desired. The application will appear onscreen.
- Complete the application: Follow the instructions and enter your information into the form fields; upload requred documents; make the payment.
- Click Next to access the application document that includes the Code of Conduct and Ethics Agreement.
- Electronically initial where required, and sign the document.
- Press complete. You can generate a PDF or print a copy of the agreement. You will also receive your signed documents via email.
- Your application documents will be sent immediately to CADTP’s system where the review process will begin.
- CADTP’s Certification Team will reach out to you if they have questions or need additional information.

Registered Counselors
Registrant Initial Registration (SUDRC)
This application is for NEW Counselors who have not previously registered with CADTP or any other certification agency.
If you are registered with another certification agency, use the transfer applications available on this page.
Registrant Renewal (SUDRC)
Complete the application to renew SUDRC registration. Required annually.

SUDCC Certification Exam
Complete this form to apply for the SUDCC exam and certification as an SUD Certified Counselor.
Complete this form to apply for a SUDCC Certification Exam Retake.
If candidates fail to show up for an examination, are late, or do not have the proper identification they will not be permitted to sit for their examination. They will be considered a “No-Show,” examination fees will be forfeited, and they will be required to re-register and pay the exam fee prior to scheduling another examination.
Complete this application if:
1. You have missed your scheduled exam date and did not reschedule prior to the scheduled exam date.
2. You have missed your 6-month period to schedule and take the exam.
3. You were late for your scheduled exam time, or you did not bring the proper identification.
