Certification Upgrade Application
How to Upgrade Your Certification:
- Have your employer complete the SUD Counseling Work Experience Form to document the required number of advanced hours for the level you are applying for. You can find the requirements for each certification level here: SUD Counselor Certification Levels.

For SUDCC III and above, obtain a copy of your transcripts or degree to show you have a higher degree in an SUD-related field. SUD-related fields include: Social Work, Psychology, Human Services, Marriage and Family Therapy, etc.

Complete the application. Ensure all form fields are filled out correctly and select the level you are upgrading to.

Upload the required documents to show you meet the certification level requirements.

Pay the non-refundable fee and sign the document.
A copy of your application will be emailed directly to CADTP, and you will receive a copy at the email address provided on this form.
